Summary

The facility failed to provide timely dental services for one resident with a history of major depressive disorder, dementia, intracranial injury, hypertension, anxiety disorder, cerebral artery stenosis, alcohol dependence, and unspecified psychosis. The resident’s annual dental exam noted full dentures and stated he was doing well with them and had no dental concerns, but later progress notes documented the resident’s mother was concerned about broken dentures and asked staff to contact the dental company. After that note, there were no follow-up progress notes addressing the resident’s dentures or dental status, even though the care plan identified full upper and lower dentures and directed staff to report dental problems needing attention. Interviews and observations showed conflicting information about the resident’s dentures and no clear evidence that the denture issue was resolved. The resident’s mother stated the dentures were broken and she was concerned about his ability to eat. An LPN later found the resident did not have dentures in his mouth, and the resident said he did not know where they were. The resident then stated he had his own teeth, but observation showed he had neither his own teeth nor dentures. Additional staff interviews revealed one staff member believed the dentures should be in the resident’s room, another had never seen him with dentures in 10 years, and a CNA could not locate them in the room. A social service designee later stated the dentures had been broken and/or missing for one year and that the resident was added to the dentist list for the next routine visit.
