Summary

The facility failed to ensure Resident #27’s respiratory needs were ordered and that oxygen equipment was cleaned and stored according to policy. Resident #27 was admitted with diagnoses including congestive heart failure, asthma, thrombocytopenia, end stage renal disease, history of pulmonary embolism, and obstructive sleep apnea. The care plan identified a potential for altered respiratory function and included administering oxygen as ordered, but the five-day MDS indicated the resident did not receive oxygen therapy and the physician’s orders showed no active oxygen order. Despite this, the resident was observed wearing a nasal cannula with oxygen running, and the oxygen tubing was dated 03/24/26. A CMA confirmed the tubing date and stated she believed it was supposed to be changed weekly, and a regional RN verified there were no oxygen orders for the resident and that oxygen tubing was to be changed weekly. The facility also failed to maintain dated oxygen equipment for Resident #89. Resident #89 was admitted with diagnoses including traumatic hemorrhage of the cerebrum, acute respiratory failure with hypoxia, and dysphagia. The resident had an order for 3 liters of oxygen with 28% humidified aerosol trach collar at bedtime and as needed, and the care plan addressed altered respiratory function related to respiratory failure and tracheostomy placement. During observation, Resident #89 was receiving oxygen at 6L with a trach mask and humidified water attached, but there was no date on the tubing, water, or trach mask showing when the equipment had last been changed. An LPN confirmed the oxygen setup was not dated at all.
