Summary

The facility failed to ensure medical records were accurate and complete for one resident. Resident #59 was admitted on 12/10/24 with diagnoses including schizophrenia, type two diabetes mellitus, and depression. The MDS assessment dated 04/01/26 indicated the resident had moderately impaired cognition and was independently mobile. Review of the progress notes from 04/30/26 to 05/18/26 showed no documentation that the resident left the facility without staff knowledge, did not sign himself out, and that his whereabouts from 05/01/26 to 05/18/26 were unknown. The facility investigation dated 05/01/26 stated the resident could not be located at about 7:50 P.M., prompting staff to search and activate the missing resident protocol, including contacting the local police department. The SRI dated 05/04/26 also stated the resident left the facility and had not returned, and that the facility searched the building and surrounding area and contacted police. Staff interviews confirmed there were no progress notes documenting the incident, and the ADON stated there should be a detailed progress note in the medical record regarding any leaves of absence from the facility.
