The facility failed to ensure adequate supervision and implementation of identified interventions for two residents who were reviewed for accidents. The deficiency involved one resident who required two-person assistance for bed mobility and another resident who was severely cognitively impaired and required close supervision for ambulation. The resident was documented as needing close supervision for ambulation, yet was found on the floor in the north hallway after apparently walking independently and losing balance. The incident was unwitnessed, and the resident reported going to the dining room and slipping and falling. Interviews with staff and rehabilitation personnel indicated the resident should not have been walking alone and should have been monitored or redirected, but the LPN who discovered the resident stated they did not know the resident’s care plan and did not know whether someone had been with the resident in the day room. The resident sustained a fractured right hip.
