Summary

The facility failed to ensure that residents were free from unnecessary drugs for three residents reviewed. Resident #231, who had diagnoses including paraplegia, Parkinsonism, and neuromuscular dysfunction of the bladder and was documented as cognitively intact, had an order for oxycodone 5 mg, 1.5 tablets by mouth once daily as needed for pain with no documented end date on the Medication Administration Record. Resident #31, admitted with anxiety disorder, quadriplegia, and polyneuropathy and also documented as cognitively intact, had an order for oxycodone 5 mg, 5 mg by mouth every 8 hours as needed for pain with no documented end date on the Medication Administration Record. Resident #13, admitted with atherosclerosis, major depressive disorder, and status post vascular surgery of the right lower extremity, was documented as usually able to understand and be understood with intact cognition. The resident had orders for oxycodone 5 mg twice daily and oxycodone 5 mg twice daily as needed, both with no end date, as well as Lexapro 10 mg daily with no end or renewal date. The record also showed repeated administration of oxycodone on multiple dates with a pain score of zero. During interviews, nursing staff stated the narcotics were reviewed as needed and that no end date was entered because the resident was expected to be in subacute rehabilitation for a short time; the nurse practitioner stated narcotics were prescribed based on individual needs and would not necessarily have an end date.
