Summary

The facility failed to provide sufficient nursing staff to meet the needs of all 89 residents, and residents reported that baths and showers were often missed because staff said there was not enough help available. Several residents stated they regularly missed scheduled showers, including on weekends, and multiple CNAs and LPNs confirmed that the facility was short-staffed with both CNAs and nurses. One CNA stated resident baths and showers can take 15 to 45 minutes, which affects staffing on the unit floors, and the DON stated it was her expectation that nursing staff offer baths and showers according to the facility’s shower schedule. The facility also failed to ensure safe Hoyer lift transfers for a resident with significant mobility needs. The resident was admitted with morbid obesity, muscle weakness, need for assistance with personal care, and traumatic brain injury. The resident’s PT note stated the resident’s mother had been instructed on proper Hoyer lift use for safe transfers and was reminded that two persons are required for all transfers. The care plan stated the resident required assistance from two staff for Hoyer lift transfers and did not indicate that family could assist instead of facility nursing staff. During observation, a CNA brought a Hoyer lift into the resident’s room alone, and later stated she asked the resident’s mother to assist because no other staff were available. The resident stated there was not enough staff to transfer her when needed and that she had to wait extended periods of time for transfers. Multiple staff members stated they were not aware of any family member trained to assist with resident transfers and stated only two trained staff members should perform Hoyer lift transfers. The DOR stated the mother’s training was provided for expected discharge home and was not intended to replace staff assistance while the resident remained in the facility.
