Record review of an undated facility policy titled "Posting Direct Care Daily Staffing Numbers" stated that the daily posting should include the facility name, current date, resident census at the beginning of the shift, the 24-hour shift schedule, the shift being posted, the type and category of nursing staff working during that shift, the actual time worked during that shift for each category and type of nursing staff, and the total number of licensed and unlicensed nursing staff working for the posted shift. Observation on 05/19/26 at 3:20 PM and 05/20/26 at 3:25 PM showed that the posted nursing staffing information for 05/18/26, 05/19/26, and 05/20/26 did not contain the facility census, a calculation of the number of hours worked per discipline per shift, or the total number of hours worked per shift and daily. Review of 30 days of posted nurse staffing documentation from 04/18/26 through 05/20/26 showed the same omissions. During interview on 05/21/2026 at 8:34 AM, the DON confirmed that the facility census was not included on the past 30 days of posted nurse staffing information and that no calculation of hours worked per discipline or total hours worked had been completed or documented.

Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
