The assaulted resident sustained visible facial injuries, including two black eyes, scratches under the eyes, an injury/marking on the nose area, an abrasion on the right forehead, and red marks on both cheeks, and was sent to the hospital for evaluation and treatment. The record also contained conflicting resident statements about who struck first, but the facility investigation substantiated a resident-to-resident physical altercation with injury. The assaulted resident later stated the other resident hit him/her, knocked him/her out of the chair onto the floor, and that staff pulled the other resident off. The LPN stated he manually removed the stool in the bathroom, did not complete an abdominal assessment, did not administer an enema before manual removal, and did not contact the provider for further direction. The report also states the facility failed to ensure the resident was free from abuse during cares, and the immediate jeopardy was identified based on this event.

She also stated she had concerns that the facility was not completing the ordered dressing changes and that the resident had worsening buttocks skin breakdown, while the DON stated treatments should be completed as ordered and documented on the TAR or in progress notes.
