Infection Control and TB Screening Failures

Summary

The facility failed to follow enhanced barrier precautions and acceptable infection control practices during wound care for two residents with wounds. One resident had paraplegia, diabetes, three stage 3 pressure ulcers, and one stage 4 pressure ulcer. During observation, a CNA entered the room without performing hand hygiene and provided care while wearing gloves but no gown. The resident had a left hip bandage with drainage soaked through the dressing and a left ankle wound with brown and bloody drainage. The wound nurse later stated the ankle dressing had serosanguinous drainage and that the wound had been debrided the day before, which would increase drainage. During another wound care observation for the same resident, the ADON and wound nurse entered the room wearing gloves and gowns, but the ADON placed a towel that had been carried on her shoulder directly under the resident's foot. The wound nurse removed the soiled dressings and the resident's foot rested directly on that towel. The ADON then covered the resident with a blanket that had noticeable dried blood on it, and the wound nurse changed gloves and resumed treatment. The ADON stated she expected enhanced barrier precautions to be used for residents with wounds and said the towel carried on staff's shoulder was contaminated and should not have been used because staff hair had touched it. The facility also failed to complete TB screening as required for another resident. The resident had an unstageable left heel ulcer and Alzheimer's disease with moderately impaired cognition. The immunization record showed the first TB skin test was administered and negative, but the second step was not recorded. The facility policy required all new admissions to have a Mantoux skin test within 48-72 hours of admission and a second test 1-3 weeks later if the first was negative.
