Summary

The facility failed to ensure consultant pharmacist recommendations from monthly drug regimen reviews were acted upon in a timely manner for a resident with intact cognition who had diagnoses including panic disorder, generalized anxiety disorder, atrial fibrillation, and peptic ulcer. The resident’s orders included meloxicam, Eliquis, Jardiance, and buspirone. The pharmacist’s 3/31/26 review noted meloxicam should be given with food or milk, Jardiance was ordered for DM2 even though diabetes mellitus was not listed among the resident’s diagnoses, and buspirone and Eliquis were associated with diagnoses that were not appropriate for those medications; the pharmacist recommended clarifying the diagnoses and updating the MAR. A later 4/29/26 review repeated the same concerns about Jardiance and the need to verify and add the diagnosis to justify use. The MAR for April and May 2026 continued to list Eliquis for heart, Jardiance for dm2, and buspirone for psych, and the MAR lacked administration instructions for meloxicam. The facility also failed to develop and/or implement a procedure that outlined follow-up time frame expectations in the drug regimen review process for another resident with intact cognition and diagnoses of respiratory failure and debility. That resident had an order for nystatin powder applied to the groin in the morning without an end date. The pharmacist’s 3/30/26 review stated antifungals should be discontinued when healing was complete and recommended discontinuing the nystatin powder, but the review did not include a date by which a response was required. The review showed no response by 4/30/26. During interview, the CP stated the facility did not have a laid out time frame for nonurgent recommendations and typically expected a provider response within 60 days, while the DON stated recommendations affecting resident safety should be addressed as soon as possible but did not provide a timeframe for nonurgent recommendations.
