Summary

The facility failed to ensure the comprehensive care plan for R128 was accurate and updated to reflect current transfer, mobility, ambulation, and toileting needs. R128’s quarterly MDS indicated independence with toileting and continence of bladder, while other documentation identified functional bladder incontinence, occasional bowel incontinence, and a check-and-change toileting schedule. The care plan also contained conflicting entries showing R128 as independent with toileting, needing toilet transfers marked not applicable because of pull-up briefs and barrier cream use, needing encouragement to toilet during rounds, having an independent toileting schedule, and requiring set-up/clean-up assistance for toilet hygiene. Staff also stated R128 knew when toileting was needed but could not independently get to the bathroom, and that the resident had fluctuating function after hospitalization. The facility also failed to ensure two care conferences were completed to correspond with R11’s MDS cycle. R11’s significant change MDS identified intact cognition, need for hygiene assistance, no rejection of care, and diagnoses including paralysis on the dominant side from a stroke, diabetes, and depression. R11 stated she did not recall having a care conference since admission and wanted to discuss discharge or transfer. The administrator stated there was no documentation supporting a care conference for R11’s quarterly and significant change MDS cycle, and staff interviews confirmed the expectation that care conferences occur with each MDS cycle. The record showed only one care conference at admission and no documentation for the quarterly or significant change MDS care conferences.
