Resident dignity not maintained during personal care and delayed call light response
Summary
Resident dignity was not maintained during personal care for a resident with severely impaired cognition, dependence on staff for personal hygiene, frequent bowel and bladder incontinence, and diagnoses including Alzheimer’s disease, dementia, bipolar disorder, and overactive bladder. During wound care and incontinent care, the resident’s door was left partially open, there was no privacy curtain, the resident’s pants were down to the ankles, the incontinent brief had been removed, and the resident’s exposed bottom was visible from the doorway. While the resident remained exposed, a housekeeper knocked, announced himself as laundry, and entered the room to place clean clothes in the closet without being stopped by the nurse or nursing assistant providing care. A second resident who was cognitively intact and required substantial to maximum assistance with most ADLs was also observed receiving incontinent care in bed while exposed. The nursing assistant removed the resident’s oxygen, turned off the oxygen concentrator, removed the blanket, and provided perineal and skin care while discussing the resident’s skin condition and treatments. During this care, multiple staff entered the room without waiting for permission, including the Administrator, who knocked and immediately entered while the resident remained uncovered from the waist up and wearing only a brief, as well as an LPN and an RN who also entered without waiting for a response. The facility also failed to maintain dignity for two residents who experienced prolonged waits for call light response and had bowel or bladder accidents while waiting for staff. One resident with intact cognition, mobility limitations, anxiety, COPD, respiratory failure, fibromyalgia, opioid abuse, pulmonary fibrosis, and oxygen dependence reported pressing the call light for help with toileting but not receiving assistance in time and having a bowel accident on the floor before reaching the bathroom. Another resident with intact cognition, frequent urinary and stool incontinence, an unhealed pressure ulcer, MS, heart failure, diabetes, obesity, and stroke-related lower-limb paralysis reported sitting in a soiled brief while waiting for staff after pressing the call light. Call light logs showed waits of 38 minutes and 53 minutes for one resident and 58 minutes and 15 seconds for the other.
