Summary

The facility failed to provide safe, appropriate dialysis care for a resident who required hemodialysis three times weekly. The DON acknowledged the resident did not have pre/post dialysis assessments completed on one dialysis day and that several assessments were not accurate because they did not contain current weights or vital signs.
