Summary

The facility failed to obtain vision services for one resident who was admitted with diagnoses including COPD, diabetes, legal blindness, and the presence of an artificial eye. The resident’s MDS showed she was cognitively intact, had impaired vision without corrective lenses, and was her own responsible party. During observation, she stated that about one month earlier she had asked to be placed on the dental and vision services lists, reported that her left eye was a prosthetic, that she had 20 percent vision in her right eye, and that she needed glasses but did not have any. The resident’s care plan identified visual impairment related to legal blindness and a prosthetic left eye, and an intervention dated 2/11/26 directed staff to arrange an eye care consultation as needed. An MD history and physical note reflected that the resident wanted to see the optometrist and dentist. However, the SW reported that the vision services provider visited every four to six weeks and was scheduled to visit on 5/11/26, but the resident was not on the list for that visit. The SW stated she had not been notified of the resident’s request and no order had been placed in the medical record. The DON and RDC reported that after the facility changed provider groups on 3/1/26, ancillary services such as vision services required an order in the medical record before the SW added the resident to the list, and that providers had not been adding the orders to the medical record.
