Inadequate Staffing, Delayed Care, and Resident Altercation

Summary

The facility failed to provide enough nursing staff each day to meet resident needs and to have a licensed nurse in charge on each shift. Survey findings described inadequate staffing on both day and night shifts, with repeated instances of too few CNAs assigned compared with the facility’s staffing plan and facility assessment. The report also documented resident complaints and staff statements that call lights were not answered promptly, residents waited extended periods for toileting and incontinence care, and resident sleep/wake preferences were not accommodated because residents were awakened early to complete care and transport needs. A resident-to-resident physical altercation occurred when one resident entered another resident’s room and sat on the resident’s bed while the resident was in it. Staff observed the injured resident tearful with visible markings on the face, scratches with blood, bruising on the face and hands, and later additional bruising on the hands, arms, face, forehead, head, and lower extremities. The injured resident was transferred to the ER. Staff interviews indicated that only one aide was assigned to the hall at the time, the aide was on break, and the LPN was covering the hall while also trying to complete nursing duties. Multiple staff members stated the hall was understaffed and that the altercation could have been prevented if an aide had been available to cover the hall. The staffing records showed numerous shifts with fewer CNAs than the facility assessment required, including 39 day shifts and 20 night shifts between 3/10/26 and 5/25/26. On the night shift, staff reported only 3 CNAs were working for 76 residents, with one CNA responsible for two halls and call lights observed illuminated while residents still needed to be gotten up and changed. Residents and family members reported long waits for assistance, including waits of 30 to 45 minutes for call lights and bathroom assistance, and one resident stated he was awakened at 4:30 AM several times per week so he could be ready for dialysis because of the lack of staff. Resident council minutes also documented ongoing concerns about night call light wait times and overworked nurses covering multiple halls.
