The facility failed to ensure cephalexin was administered as ordered for one resident with Parkinson's disease, diabetes, and peripheral vascular disease who was cognitively intact on the MDS. The resident was admitted with cellulitis of the right lower extremity and was observed sitting in a wheelchair with the right leg wrapped in a bandage while reporting she was receiving an antibiotic for the condition. The physician ordered cephalexin 500 mg three times daily for 7 days, but the MAR did not show administration for multiple scheduled doses, including doses on 3/4/26, 3/6/26, 3/7/26, and 3/10/26.
