Summary

The facility failed to have an active and ongoing infection prevention and control plan for reducing the risk of legionella and other opportunistic pathogens of premise plumbing. During interview, the Facilities Operations Manager stated that housekeeping staff flush the domestic water lines in soiled utility rooms that are not used regularly, but there were no other rooms or fixtures identified for regular flushing. The Facilities Operations Manager also stated that the facility was not performing any sampling of the water for disinfection or legionella. Observation of the [NAME] Ridge spa room found the spa tub disconnected from the water supply and moved to the far side of the spa, and the shower fixture had been removed, indicating it was not used regularly. Maintenance staff stated that the water lines run through the ceiling in this area, and the Facilities Operations Manager stated that staff and residents do not use this spa for showering and bathing residents and instead take residents of that hall to another spa. When asked if these fixtures were on a flushing schedule, the Facilities Operations Manager stated no. Observation of the Red Oak spa tub room found the room being used for storage of boxes and supplies, with the commode and sink portion not accessible for use or easy flushing of the water fixtures. The spa tub had been disconnected from the water supply, and the Facilities Operations Manager stated it had been disconnected for a while. Review of the facility's Water Management Program - Legionella, last revised on 4/15/2026, stated that the purpose of the plan was to provide guidance on recognition, evaluation, and control of Legionella and to verify and validate that control measures are effective, and included statements to eliminate dead legs, avoid stagnation, and check residual chlorine to ensure proper disinfection is available.
