Summary

The facility failed to complete wound care as ordered for two residents with pressure-related wounds. Resident #10 was admitted with chronic kidney disease and had multiple pressure wounds to the right hip, left hip, and coccyx. During observation, the resident stated that a wound on his buttocks was supposed to be changed daily but had not been changed for 2 to 3 days. When the dressings were later observed with an LPN, all three dressings were clean, dry, and intact, but none were dated, and the LPN stated she had changed them that day and normally dates dressings but could not find a marker. Resident #13 was admitted with diagnoses including type 2 diabetes, peripheral vascular disease, and acute osteomyelitis of the right ankle and foot. The resident stated that his left foot dressing was ordered to be changed every day but was instead being changed about every 2 days. When the dressing was observed with an RN, it was clean, dry, and intact, but no date was present. The RN stated she had completed the dressing change that same day and that she typically dates each dressing. The wound NP documented repeated concerns that wound care was not being completed regularly or according to orders for both residents. For Resident #10, the wound NP noted that dressing orders needed adherence and that wound care must be done regularly. For Resident #13, the wound NP documented that dressings had not been changed for extended periods, including one note stating the dressing had not been changed in one week, and repeatedly wrote that dressings must be changed regularly. During interview, the wound NP stated that there were times when wound care had not been performed or not performed as ordered, and nursing administration confirmed that no audits beyond weekly wound rounds were completed and that no ongoing or widespread education had been done after being notified of missed wound care.
