Summary

The facility failed to ensure that a resident who required a mechanical lift for all transfers was provided with equipment specifically designed for safe use during the transfer. Resident #3, who had spinal stenosis, lumbar disk degeneration, dementia, and atrial fibrillation, was care planned to require two staff members and a mechanical lift for all transfers. The facility policy stated that manufacturer recommendations would be followed when using the mechanical lift, and the lift manual indicated not to use a sling unless it was recommended for use with the lift. On 04/18/26, nursing staff attempted to transfer Resident #3 using the mechanical lift with a repositioning device rather than a designated lift sling. Staff interviews indicated the device was found in the laundry room, lacked the longer leg straps that crisscross between the resident’s legs, and was later identified by the Assistant Director of Nurses as a repositioning pad not intended or designed for use as a mechanical lift transfer sling. CNA staff stated they recognized the device seemed different from usual lift slings, but it was still used for the transfer. The resident was transferred from bed to wheelchair earlier in the day and later from wheelchair back to bed using the same device. During the evening transfer, Resident #3 slid out of the mechanical lift, fell to the floor, struck his/her head, and sustained skin tears. Nursing staff documented that the resident reported hitting his/her head and was sent by 911 to the Hospital Emergency Department. Hospital imaging showed an 8-mm intraparenchymal hemorrhage in the left occipital lobe of the brain. The facility investigation concluded the fall occurred because staff used an improper device in place of a transfer sling specifically designed for the mechanical lift.
