Failure to Maintain Required Source Control Masking

Summary

The facility failed to ensure staff appropriately wore source control face masks during a period of increased influenza in the community. On 1/22/26, the survey team entered the facility and the night nurse supervisor reported that due to the high flu rate in the county, face masks were currently required. The report states that on 12/31/25 the Maryland Department of Health issued recommendations for healthcare facilities to implement facility-wide source control measures in patient care areas and other patient-facing settings, including requiring all individuals such as clinical staff, non-clinical staff, and visitors to wear masks in those areas. During six random observations on day one of the survey, multiple staff members were observed with masks worn incorrectly or not worn at all in patient-facing areas. A GNA was observed with a mask below the nose and stated it was a little bit suffocating. Another GNA was observed in the hallway with the mask not covering the mouth and nose, later without a mask in the hallway and elevator while residents were present, and then providing care to a resident with the mask not covering the mouth or nose; the GNA acknowledged the mask should be worn over the mouth and nose and said it was 80 degrees in the room. An environmental service staff member was also observed with the mask below the nose and mouth and stated awareness of how the mask should be worn. The Infection Preventionist later acknowledged awareness of the Maryland Department of Health notice and confirmed that staff on the floor were expected to be masking in patient care areas, including hallways.
