Summary

The facility failed to ensure that concerns identified through the QAPI process were addressed through the development of a Performance Improvement Project (PIP). During an interview, the DON explained that the facility met monthly to discuss concerns identified by the QAA committee and that the process was data driven, and she identified annual staff training as one of the current concerns being discussed in QAPI. Record review showed performance evaluations for GNA #25, GNA #26, and GNA #27, with GNA #25 and GNA #26 receiving a score of 1, defined as Performance Below Standard, and comments noting Relias 0% and a need to complete Relias training. Training records also showed multiple instances of incomplete annual trainings. When the NHA was interviewed about the gaps in employee education, she acknowledged that there was an issue with staff completing required courses. In a later interview with the NHA, DON, ADON, and HR, none of the four staff members could identify who was responsible for ensuring all facility staff completed required annual training, and the NHA and DON agreed there was a systemic gap in ensuring staff completed annual education. During a subsequent discussion, the DON stated the committee was aware of the concern and had discussed it, but confirmed the facility had not developed a PIP to analyze the causes or implement solutions related to the lagging education. The DON and NHA acknowledged that a formal PIP would be expected when an issue is identified by the quality assurance committee.
