Summary

The facility failed to ensure resident medical records were accurately documented and maintained according to professional standards for advance directives and medication-related care planning. For two residents, the social services assessment documentation indicated they did not have an advance directive, that additional conversation about advanced care planning was not provided, that the opportunity to complete an advance directive was not provided, and that advance directive educational materials were not provided. The Director of Social Services stated that residents were asked about advance directives and that materials would be offered if they did not have one, but also stated the template was confusing and could be interpreted several different ways when documenting. For another resident, the medical record contained an active physician order for monitoring related to anticoagulant/antiplatelet medication use, including observation for bleeding and other adverse signs, even though the resident did not have an active physician order for an anticoagulant or antiplatelet medication. The record also showed a care plan goal stating the resident would remain free of complications related to anticoagulant therapy, with interventions to give medications as ordered and monitor for side effects and signs of abnormal bleeding, bruising, and petechiae related to anticoagulant use. During interview, staff identified the interdisciplinary clinical team as responsible for the care plan process. The DON was informed that the resident was not currently receiving an anticoagulant or antiplatelet medication, yet the active physician orders and care plan still reflected monitoring for complications related to those medications. The DON acknowledged the concern and stated she would review and follow up, and no additional information was provided at survey exit.
