Summary

The facility failed to ensure a resident’s medical record was complete and accurate by not properly documenting administration of narcotics on the narcotic record for 2 of 2 medication carts reviewed. The facility policy dated 01/15/2026 stated that the charge nurse who administers a prescribed narcotic must immediately document it on the MAR and the controlled drug administration record. During review of Medication Carts A and B on 03/25/2026 at 2:15 p.m., discrepancies were identified between the narcotic records and the medication cards for multiple residents. For Resident #36, Hydrocodone/APAP 10-325 mg was documented on the MAR as given at 08:48 a.m. by S5 LPN, but the narcotic record had not been signed at the time of review, and the count showed 18 tablets remaining on the card when the record reflected 19. Similar count discrepancies were found for Resident #67, Resident #79, Resident #30, Resident #4, Resident #25, Resident #46, and Resident #56, with narcotic records showing one more tablet or capsule remaining than was actually present on the medication card. S5 LPN stated she gave the medications but did not document them on the narcotic record, and the DON confirmed nurses should document narcotics at the time they are administered.
