Missed nephrology appointment due to van having no gas
Summary
The facility failed to ensure a resident received treatment and care in accordance with professional standards of practice when a scheduled nephrology appointment was not transported to as ordered. The resident had an admission date of 02/07/2025 and diagnoses that included chronic embolism and thrombosis of unspecified deep veins of the lower extremity, bilateral, type 2 diabetes mellitus with diabetic neuropathy, peripheral vascular disease, and benign prostatic hyperplasia without lower urinary tract symptoms. The resident’s quarterly MDS with an ARD of 04/15/2026 showed a BIMS score of 14, indicating intact cognition, and the resident used a wheelchair for mobility. The resident’s monthly physician appointment calendar showed a nephrology appointment scheduled for 05/22/2026 at 8:20 a.m., but the appointment was crossed out with the note, “No Gas.” In interview, the resident stated he was waiting to leave around 6:00 a.m. for the out-of-town nephrologist appointment, but after the scheduled departure time passed, he asked the nurses’ station when he was leaving and was told the appointment had been rescheduled because the facility van had no gas. The resident stated he was upset, said it was not his fault, and reported that his daughter was also upset because he now had to wait another 3 to 4 weeks for the rescheduled appointment. Staff interviews confirmed the transportation failure. The driver stated the van did not have enough gas to take the resident to the appointment and that the gas card was locked in the administrator’s office, making it too late to obtain gas. Maintenance and nursing staff stated drivers had the ability to put gas in the vans and that the appointment should not have been missed for that reason. The DON and administrator both confirmed the missed nephrology appointment was unacceptable, should not have happened, and delayed the resident’s treatment; the administrator also stated the drivers were responsible for fueling the van the day prior and that the resident’s specialist appointment was delayed by 4 more weeks because of the facility’s transportation planning.
