The facility failed to follow R4’s care plan for transfers when staff used a sit-to-stand lift instead of the ordered full-body mechanical lift. R4 had diagnoses including hypotension, dementia without behavioral disturbances, type 2 DM, chronic kidney disease, and hypertension, and the MDS documented that R4 was dependent on staff for transfers, lower body dressing, and toileting hygiene.
