Summary

The facility failed to ensure residents were free from significant medication errors when an LPN administered 30 units of Lantus insulin to a resident who was not diabetic. The resident’s EMR documented dementia, and her MDS assessments documented severe cognitive impairment with BIMS scores of four and three. Her records also showed no diagnosis of diabetes mellitus and no insulin use during the seven-day lookback periods. Her care plan instructed staff to administer medications as ordered. The EMR progress note documented that the LPN gave the insulin into the resident’s left upper arm and immediately realized it had been given to the wrong resident.
