Failure to Use EBP and PPE During Wound Care

Summary

The facility failed to ensure Enhanced Barrier Precautions (EBP) were in place for a resident with a Stage 3 pressure ulcer to the sacral area and failed to ensure staff used appropriate PPE during wound care. On 04/15/26, an Administrative Nurse entered the resident’s room without a gown, and a CNA was already present with gloved hands but also without a gown. No EBP sign was noted in the room or on the door. The CNA raised the resident’s bed and pulled down his pants and untabbed his brief while the wound care supplies were set up on the bedside table. During the dressing change, the Administrative Nurse removed the old dressing, discarded it, removed and replaced gloves multiple times, washed hands at the sink between steps, and applied saline-soaked gauze, Santyl, collagen particles, and a new dressing to the wound, but did not wear a gown at any point. The nurse stated the wound was new and not chronic, so EBP and a gown were not needed, and later stated the facility followed CDC guidance and that she understood EBP was only needed for chronic wounds. Another Administrative Nurse stated it was her understanding that full EBP, including a gown, was not needed unless a wound was chronic. The facility policy dated 04/01/24 stated EBP would be ordered for residents with wounds, including chronic wounds such as pressure ulcers, and that gowns and gloves would be made available immediately near or outside the resident’s room.
