Failure to Reconcile Hospital Pain Medication Orders

Summary

The facility failed to prevent a medication error when it did not reconcile hospital pain medication orders for a resident with chronic low back pain, metastatic bone disease, prostate cancer, and type 2 diabetes. The resident had intact cognition with a BIMS score of 14 and was receiving scheduled and PRN pain medication. After returning from the hospital, the resident’s hospital paperwork listed scheduled oxycodone 20 mg every 8 hours and pregabalin 300 mg twice daily, along with PRN hydromorphone for cancer-related pain, but the facility’s EMR did not show that the scheduled oxycodone and pregabalin orders were reconciled or started as ordered. The resident arrived back at the facility and repeatedly reported severe pain and withdrawal concerns. Nursing notes documented that he asked where his medications were, stated he had withdrawals, and later requested transfer to the hospital for acute pain in his legs, back, and chest with chills and sweating. The resident received PRN hydromorphone for pain rated 9 out of 10, but the MAR showed he did not receive the hospital-ordered scheduled oxycodone 20 mg every 8 hours until several days later and did not receive pregabalin 300 mg twice daily until several days later. During that time, the MAR documented pain ratings of 7 to 10 out of 10. Record review showed no documentation explaining why the hospital’s scheduled oxycodone and pregabalin orders were not reconciled or initiated on readmission. Interviews with nursing and administrative staff confirmed the discrepancy was not caught during the admission process. One nurse stated the orders were being verified, and administrative staff stated the hospital orders should have matched and that the discrepancy should have been identified within 24 hours, but it was not. Another administrative nurse stated the missed scheduled oxycodone and pregabalin doses were medication errors and that the hospital orders had not been reconciled when the resident returned.
