Failure to Separate Residents After Threats and Assault

Resident #12 had diagnoses including hypertension, CVA, diabetes, seizure disorder, anxiety, depression, acute pain due to trauma, fractures, and substance abuse history. Her MDS showed mild cognitive impairment with a BIMS score of 11 out of 15, and she required maximal assistance for repositioning, transfers, dressing, toileting, and hygiene, and was unable to stand or ambulate. During breakfast, Resident #11 approached Resident #12 in the dining room and struck her in the head with a water cup or thermos mug while staff were present in the area. Resident #12 sustained a bruise on her face/head area, voiced that she did not feel safe, and requested transfer to another facility.
