Summary

The facility failed to ensure a resident representative was notified of new medication orders for Resident 19 and failed to obtain informed consent before initiating or increasing psychotropic medications for Residents 19, 76, and 7. Resident 19 had moderate cognitive impairment and was started on Rexulti for agitation/anxiety, had trazodone increased for insomnia, and later received haloperidol stat for anxiety and agitation. Resident 7 had severe cognitive impairment and Alzheimer’s disease; lorazepam was increased from twice daily to three times daily for delusional disorders, and the record lacked documentation of informed consent from the resident’s representative. For both residents, the order change progress notes identified black box warnings but left the responsible party notification sections blank.
