Summary

The facility failed to ensure wound treatments were completed as ordered for two residents with pressure-related skin conditions. For one resident, the clinical record showed a right ischium pressure ulcer that began as a Stage 1 injury and progressed to an unstageable wound with eschar and slough. The resident had dementia and diabetes and was severely cognitively impaired. Although an order was in place for the wound to be cleansed, packed with calcium alginate, and covered with bordered gauze, the February 2026 EMAR/ETAR lacked documentation that this treatment was completed during the ordered period. During observation, the Assistant Director of Nursing prepared and completed the wound treatment for the first resident, and the wound was noted to be approximately tennis-ball size with measurable depth, slough, and gray tissue. Record review showed the wound had worsened over time from a small dry Stage 1 pressure ulcer to an unstageable pressure ulcer with drainage and tissue breakdown. The ADON stated the resident had a rapid decline and that the treatment should have been checked off in the EMAR/ETAR when completed, but the record did not show that the calcium alginate treatment was documented as administered as ordered. For the second resident, the wound on the right buttock/lower back area was observed as a circular wound about 5 cm in diameter and 2 cm deep, with a pink wound bed and no drainage or signs of infection. Record review showed the area began as incontinence-associated dermatitis and later developed into an unstageable pressure ulcer with slough and eschar. The resident had intermittent incontinence, slept in a recliner, and refused some pressure-relieving interventions. The February 2026 EMAR/ETAR showed the wound treatment was changed from medical grade honey to a zinc oxide-based hydrophilic paste mixed with collagen particles, but the record lacked indication that the treatment should have been changed on that date, and the treatment documentation did not align with the wound progress notes showing honey was to continue daily.
