Summary

The facility failed to identify pressure ulcers before they advanced for 2 of 3 residents reviewed for pressure ulcers. One resident had diagnoses including heart failure, a non-pressure chronic ulcer of the left heel and midfoot, spinal stenosis, and right leg pain, and was assessed as cognitively intact but needing partial to moderate staff assistance with transfers, lower body dressing, and footwear. The resident was also identified as at risk for pressure ulcers. During observation and interview, the resident reported right heel pain during transfers with a sit-to-stand mechanical lift and stated a heel protector was used at night because of a prior sore on the right foot. A dark-purple, non-blanchable area measuring 3.5 x 3.5 cm was later observed on the right heel and was painful to touch. Staff interviews showed the right heel area had not been reported or documented before it was observed by the surveyor and nurse staff. The RN stated there were no new orders for the right heel and no report from aides after the resident’s shower. Another RN stated she had done wound rounds the day before and had not seen anything on the right heel, and this was the first time she had heard of it. The NP observed the area, noted it was non-blanchable and mushy underneath, and stated it could not be staged because it was closed. The facility’s policy defined a deep tissue injury as a purple or maroon localized area of discolored intact skin due to pressure or shear, and the DON stated the area sounded like a DTI. The resident’s record did not show documentation of the right heel wound until after the NP assessment. The second resident had Parkinson’s disease, malnutrition, a stage 4 sacral pressure ulcer, and a stage 2 right heel pressure ulcer, and was assessed as cognitively intact, requiring maximum assistance with transfers and being at risk for pressure ulcers. The wound record showed a new right heel pressure ulcer measuring 1 x 0.9 x 0.1 cm with moderate exudate and exposed dermis. The resident was observed sitting in a wheelchair during meals and activities, and a CNA stated the resident stayed up in the wheelchair each day and only got into bed in the late afternoon. The DON stated skin checks should be done every time a resident is given care and said the heel ulcer should have been found before it became stage 2. The wound physician also stated the resident was at high risk and a good skin check should have found the new area before it advanced to stage 2 or above.
