Failure to Maintain Aseptic Technique During Wound Care

Summary

The facility failed to maintain aseptic technique during wound care treatment for two residents, including one resident with multiple sclerosis, chronic wounds, neuromuscular bladder dysfunction, anemia, and moderate cognitive impairment, and another resident with Parkinson’s disease, diabetes, respiratory failure history, and dependence for multiple activities of daily living. Both residents had physician-ordered wound treatments and were observed during wound care by the wound physician, the Administrator, and the DON. For the first resident, the wound physician was observed performing wound care while wearing gown and gloves, but repeatedly touched items and moved between wound sites without doffing gloves, performing hand hygiene, or changing gloves. The physician picked up the resident’s television remote with gloved hands, handled wounds on multiple body areas, and continued care after touching the remote and after handling scissors that fell to the floor. Fecal matter was observed on the resident’s incontinent pad and in the crevice of the buttock before wound care was completed, and the physician removed dressings, measured wounds, debrided areas, and applied dressings without consistent glove changes or hand hygiene. The physician also placed bandage scissors in the gown, dropped them to the floor, picked them up, and continued care without hand hygiene or new gloves. For the second resident, the wound physician, Administrator, and DON were observed in the room performing wound treatments while wearing disposable gowns and gloves. The physician removed a dressing, left the room to the treatment cart for supplies, and returned to apply dressings to multiple open areas, repeatedly changing gloves without observed hand hygiene and retrieving gloves from a pocket. The physician instructed staff to prepare additional dressings, applied dressings to several wounds, and later removed scabs from the resident’s heels while continuing the assessment. The facility’s wound care policy required handwashing before and after glove use and clean technique during dressing changes, and the infection prevention policy required hand hygiene before and after each care contact, but these practices were not followed during the observed wound care events.
