Serenity Estates Of LincolnshireLincolnshire, Illinois Survey Completed on 05-13-2026

Summary

The facility failed to be administered in a manner that ensured supplies were available for dependent residents, affecting 1 of 3 residents reviewed for administration in a sample of 7. On 5/11/26 at 1:15 PM, R2 was observed in bed wearing a t-shirt and an incontinence brief. R2 stated the facility had no briefs and had been out for a couple of days. R2 said he used green XL briefs because they were the most comfortable, but he had only a few left in his closet and had been wearing them wet to make them last longer. R2 said the briefs were uncomfortable when wet, but he had to make them last. His closet did not contain any incontinence briefs. At 1:35 PM, a CNA stated the facility was out of the green XL briefs and had been out over the weekend as well. The CNA said there were a few pull-up briefs available, but not the type R2 used, and showed the unit supply closet, which contained only a few pull-ups and three white small-size briefs. At 1:55 PM, the DON said a supply order was in and expected to be delivered Wednesday, and that supplies were ordered based on census and need. She also said if the facility ran out of something, they could go to a local store for supplies, and stated she was not aware they were out. The central supply room contained several empty pallets, one opened box of pull-up briefs with 3-4 remaining, and otherwise empty storage. The Regional Administrator stated the facility should not run out of supplies and should obtain them from local stores if needed. The facility's Central Supply Policy stated it was the policy to ensure adequate quantities of supplies were provided to ensure ongoing care of residents.
