Summary

The facility failed to prevent fall incidents and failed to provide a safe transfer using a mechanical lift for a resident with morbid severe obesity, vascular dementia, depression, anxiety, hypertension, and a BIMS score of 9/15 indicating moderate cognitive impairment. The resident had a care plan identifying her as at risk for falls due to cognitive and functional deficits, with an intervention to assess for anxiety and agitation prior to transfer. Two CNAs were assisting, with one operating the lift and the other spotting the resident, but the resident was described as heavy, hard to position, and actively flailing her arms and legs while suspended. Staff reported the mechanical lift began to tilt, the bed frame was lifted, and the resident fell onto the mattress and then to the floor with the mattress. The DON and Administrator stated the resident’s movement caused the lift to tip during the transfer.
