Failure to Identify and Accurately Assess Pressure Injuries

Summary

The facility failed to identify pressure injuries before they progressed to advanced stages, failed to accurately assess the wounds, and failed to put interventions in place to prevent further skin breakdown for two residents. One resident was admitted with chronic kidney disease, type 2 diabetes mellitus, mild cognitive impairment, and dependence on renal dialysis. Her sacral skin issue was first documented as moisture associated skin damage/incontinence associated dermatitis with slough and moderate serosanguineous drainage, but later was evaluated as an unstageable pressure injury with slough and eschar. The wound physician assessment described the sacral wound as having 11-20% adherent yellow slough and 71-80% moist black eschar measuring 6.65 x 7 x 0.1 cm. The record shows the resident had significant weight loss and poor intake during this period. Dietary review documented weight loss of 13.2% in 3 months and 12.5% in 6 months, later worsening to 20.1% in 1 month and 30.7% in 3 months, with meal intake ranging from 0-100%. The dietitian recommended renal supplement, liquid protein, Arginaid, and a renal multivitamin, but the physician's order sheet shows liquid protein and Arginaid were not ordered until 38 days after the initial dietitian recommendation and 31 days after the wound nurse practitioner’s recommendation. Staff stated the resident was incontinent of bowel and bladder, was difficult to keep clean, stopped eating and drinking, and that the wound initially appeared to be MASD before later showing pressure-related tissue damage. A second resident, admitted with hypertensive heart and chronic kidney disease, type 2 diabetes mellitus, and COPD, had an initial wound assessment showing no open wounds, but a later assessment documented a new buttocks pressure injury with partial thickness skin loss, moderate serosanguineous drainage, slough, fragile tissue, erythema, and excoriation. During observation, staff noted a large reddened area to the buttocks with an open area over the sacrum. When interviewed, the wound RN acknowledged an error in documenting slough for a stage 2 pressure injury and stated the wound had been present for an unknown length of time.
