Failure to Monitor Antibiotic Use and Document Infection Justification
Summary
The facility failed to follow its antibiotic stewardship policy for 4 of 4 residents reviewed for infection control. The report states the facility’s policy required an Antibiotic Stewardship Program with protocols and systems for monitoring antibiotic use and assessment of residents for infection using standardized tools and criteria adapted from the revised McGeer Criteria. Surveyors found that the facility continued antibiotic therapy for residents without documenting a causative organism or completing urine cultures to justify treatment, and staff stated they routinely continued prophylactic antibiotics without additional urinalysis or cultures. For one resident, the October 2025 Infection Control Log did not document a causative organism for a UTI, yet the resident received Cephalexin 500 mg twice daily for ten doses after returning from the hospital. The Administrator stated the resident came back on oral antibiotics, but no urine culture was completed to justify Cephalexin. For another resident with dementia, type 2 diabetes, a history of UTIs, and a vaginal fistula, the care plan and interventions described long-term alternating antibiotics for UTI history and prophylaxis. Orders included Bactrim DS, Lactobacillus for daily long-term antibiotic use, and later Cephalexin for prophylactic measures, and the resident continued receiving Bactrim as scheduled. A third resident, who was cognitively intact but dependent for toileting hygiene and frequently incontinent of urine, had a standing order for Cephalexin 250 mg at bedtime for UTI prevention and received it as scheduled from January 2025 through March 2026. The resident stated she had not been on antibiotics for UTIs before admission and was told she would likely remain on the antibiotic for the rest of her life. A fourth resident admitted with Parkinson’s disease, UTIs, and osteoarthritis had Azithromycin ordered three times weekly related to personal history of UTI, and the MAR showed ongoing administration. The Infection Control Log did not document a causative organism for this resident or document the Azithromycin order, and the Infection Preventionist stated the facility continued prophylactic antibiotics without doing extra urinalysis or cultures.
