Summary

The facility failed to assess and provide a plan of care for the use of enteral feeding and failed to ensure that enteral nutrition was provided via G-tube according to physician orders for three residents. For one resident with a G-tube for attention to gastrostomy, dietary documentation noted that the resident was consuming 76% of meals and recommended holding enteral feeding, while the resident stated the tube feeding needed to be cut off and that she was tired of it. The DON stated the resident did not use tube feeding for nutrition and only used the G-tube for medication administration because the resident refused medications by mouth, but the DON could not find a care plan addressing medication refusal and stated there was no policy related to determining the necessity for enteral feeding. The RD stated the tube feeding had been put on hold because the resident was obese and that the last assessment was on January 23, 2025. For two other residents with active tube feeding orders, staff did not follow the ordered enteral feeding schedules and rates. One resident had an order for Glucerna 1.2 at 65 cc/hr from 12:00 PM to 6:00 AM, but on two observations the feeding pump was turned off and no tube feeding equipment was seen at the bedside. Another resident had an order for Jevity 1.5 at 65 cc/hr for 20 hours, but the feeding container was labeled for 65 ml per hour while the pump was set at 75 ml per hour until staff corrected it. An RN stated she had programmed the pump incorrectly, and an LPN stated he usually started both residents' tube feedings around noon. Staff also stated that if the first resident's feeding was not infused per physician orders, nutritional needs might not be met and weight loss could occur, and that the second resident's feeding could infuse too fast and cause the resident to become too full or aspirate.
