Summary

The facility failed to ensure soft and bite sized foods were prepared in the correct texture before plating and serving for residents with modified diet orders. The diet order list dated 3/2/26 showed that one resident had an easy to chew diet with bite sized meats, three residents had soft and bite sized diets with ground meats, and another resident had a soft and bite sized diet. During the noon meal service on the second floor, a cook began setting up with coined glazed carrots and an unsliced pot roast, then cut the roast into large sections and later diced portions into large cubes. When the surveyor asked whether a plate with coined carrots was for a resident on a soft and bite sized diet, the cook confirmed it was, and the Food Service Director stated the carrots should have been cut into 1/2 by 1/2 by 1/2 sizes for all soft and bite sized foods, including the carrots and meats. During the meal, one resident on a soft and bite sized diet was served pot roast cut into large chunks and asked dietary staff to cut the meat smaller after placing a piece in the mouth and removing it. A test tray later showed the soft and bite sized pot roast was diced into 1/2 x 1/2 cubes on one face, but when turned on its side the longest side measured approximately 3/4 to 1 inch and was larger than two thumbnails. The SLP stated the facility had been implementing the IDDSI texture modified diet orders for about four to six weeks, that soft and bite sized foods should be cut into 1/2 x 1/2 x 1/2 sizes or smaller and be soft, and that serving food greater than the assessed limit poses a choking hazard.
