Failure to Administer Ordered IV Antibiotic and Flush

Summary

The facility failed to administer ordered medications to one cognitively intact resident with a history of total left hip arthroplasty and revision, anxiety disorder, pain related to internal orthopedic prosthetic devices, pain in the left hip, infection and inflammatory reaction due to an internal left hip prosthesis, joint derangements of the left hip, and cerebral palsy. The resident’s hospital discharge record and physician orders documented Cefepime 6 grams IV continuously for six weeks, along with Heparin Sodium Lock Flush 10 units/mL, 5 mL daily. The MAR showed the resident did not receive Cefepime on multiple days and did not receive the Heparin flush on multiple days during November 2025. Nursing documentation noted the resident reported cutting the antibiotic line to the CADD pump because it was beeping, with the PICC line still intact and the tubing in two pieces. The pump could not be restarted because no tubing/cartridge was found. Another nurse progress note documented Cefepime could not be administered because the facility did not have the tubing for the CADD pump. The DON stated the resident was not given the ordered Cefepime on several dates, that there was no documentation explaining why the antibiotic was not administered, and that the resident missed complete doses when the facility did not have the specific tubing required for the CADD pump. The DON also stated nurses should administer medications per physician order and document why a medication is held if it cannot be given.
