Unsafe Food Storage Temperatures and Missing Temperature Logs

Summary

The facility failed to ensure food was stored at safe temperatures, failed to maintain required temperature logs, and failed to discard potentially hazardous food stored above safe temperature ranges. During observation and interview, a CNA reported that a carton of milk served on a resident tray appeared clumpy, and an LPN stated the resident reported his milk had curds and lumps in it. The LPN removed the milk from the resident and told other residents to return their milk to staff. The resident stated he had been served milk that was curdy and spoiled for the first time. The dietary supervisor later verified milk temperatures at 48 degrees Fahrenheit and stated that refrigerated food should have an open date and expiration date, that cold foods should be held at 40 degrees Fahrenheit, and that foods held above 41 degrees Fahrenheit can cause residents to become sick. Surveyor observations in the dietary area found a refrigerator cooler thermometer reading 45 degrees Fahrenheit, parmesan cheese with a manufacturer date of 3/10/2026 and a handwritten date of 4/15/2026, deli ham with an expiration date of 4/11/2026, leftover turkey with an expiration date of 4/15/2026 and no use-by date, and five 10-pound packages of ground beef at 49 degrees Fahrenheit. On a later observation, the walk-in freezer internal thermometer read 34 degrees Fahrenheit while the external thermometer read 42 degrees Fahrenheit, the refrigerator measured 38 degrees Fahrenheit while the outside thermometer read 53 degrees Fahrenheit, and the freezer temperature log had no entry for 4/18/2026. Additional food items were observed not frozen, including pork sausage at 23 degrees Fahrenheit, Salisbury patties at 24.3 degrees Fahrenheit, and vegan homestyle chicken tenders at 30.9 degrees Fahrenheit. The dietary supervisor stated the ground beef had been placed in the refrigerator on 4/15/2026 for use by 4/21/2026, that staff sometimes leave the refrigerator open, and that he was not 100% sure at what temperature spoilage begins.
