Summary

The facility failed to provide enough nursing staff each day to meet resident needs and failed to have a licensed nurse in charge on each shift. The report states the facility assessment required 2 nurses on each floor per shift, with a total of 8 nurses required for the 4 floors, and facility staff confirmed that 2 nurses were supposed to be assigned to the 1st, 2nd, and 4th floors and 3 nurses to the 3rd floor on mornings and evenings. R2’s MAR showed blank entries for famotidine, metoprolol tartrate, and topiramate on that shift. The DON stated that if something is not documented, it was not done, and acknowledged that the facility normally tries to have nurses from different floors do med passes when short-staffed. Resident R8, who had a BIMS score of 14 and orders for hydralazine, metoprolol tartrate, gabapentin, and Lantus, had blank MAR entries for multiple scheduled medications on the same evening shift. Resident R9, who had a BIMS score of 15 and an order for Advair every 12 hours, stated there was no nurse assigned to the unit and that the inhaler due at 8:00 p.m. was a problem; the MAR showed blank entries for Advair on multiple dates including the evening shift in question. For R3, who had sepsis and a BIMS score of 13, the report states vancomycin was not available as ordered, with progress notes documenting that staff were awaiting pharmacy delivery and the DON later identifying 6 missed doses. The medication administration policy required medications to be given at the proper time, dose, and route, and if not given as ordered, the reason had to be documented on the MAR.
