Leftovers Not Monitored and Opened Foods Improperly Stored

The Sapphire At NorthwoodsBelvidere, Illinois Survey Completed on 05-14-2026

Summary

The facility failed to ensure a process was in place for monitoring cool-down and reheating temperatures for leftovers. On 5/14/26, surveyors observed a plastic food service container of oatmeal from breakfast sitting on the top shelf of the cooler and noted it was warm to the touch. A four-inch half-size hotel pan of scrambled eggs and a six-inch half-size hotel pan of hard boiled eggs were also in the cooler, both covered with steam table lids and both warm to the touch. The Food Service Director stated the facility did not currently use a cool-down log to track leftover temperatures and did not have a policy for monitoring cooled foods. A cook said she had not been trained on or provided a cool-down log and said she typically only reheats leftovers to 135 degrees Fahrenheit. The facility’s reheating policy stated cooked food cooled in the refrigerator and reheated for hot holding is reheated to 165 degrees Fahrenheit for 15 seconds. The facility also failed to ensure opened foods were labeled and stored in a food-safe manner. Surveyors observed an opened bag of egg beater mix placed into a pitcher without a top, with dried, crusty old egg beater mix around the opening; the label showed a date of 4/28/26. Surveyors also observed oatmeal, scrambled eggs, and hard boiled eggs in the cooler, and the oatmeal and hard boiled eggs did not have labels. The Food Service Director stated opened items should be reclosed and placed in a sealed container and that all items in the cooler should be labeled and dated. The facility’s labeling and dating policy stated foods are labeled with the date received, the date opened, and the discard date.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
