Summary

Food and drink were not provided in a palatable, attractive, or appropriately prepared manner. The facility was unable to provide a Dietary policy. The Resident Roster provided by the Administrator documented 94 residents in the facility. On 5/6/26, residents stated that meals served by the facility were not appealing and were often inedible. During lunch, the posted menu listed chicken fried rice and zucchini, but the fried rice was brown with very small pieces of diced chicken and the zucchini slices were limp and sitting in watery fluid. During the 5/8/26 lunch meal service, the dining room menu was posted with a date from a prior day and did not match the weekly menu provided by the Dietary Manager, which listed ravioli and roasted yellow squash for lunch. While observing the meal, the surveyor noted many uneaten ravioli edges remaining on plates, and residents were seen cutting off the edges around the meat-filled ravioli and eating only the middle portion. Residents stated the food was often bad, poorly prepared, and not something they would serve to anyone. They also reported burnt eggs at breakfast and hot cereal that was mostly water with only a few grains of cereal. The Administrator stated the facility was aware of the food and preparation issues in Dietary and had made staffing changes, including hiring a new Dietary Manager and interviewing additional Dietary personnel.
