Summary

The facility failed to ensure timely assessment, physician notification, documentation, and implementation of treatment interventions for pressure ulcers for one resident with severe cognitive impairment and dependence on staff for activities of daily living. The resident had diagnoses including acute respiratory failure with hypoxia, nontraumatic intracerebral hemorrhage, encephalopathy, gastrostomy status, tracheostomy status, and type 1 diabetes. The facility policy required admission skin and pressure ulcer risk assessments, prompt reporting of identified skin conditions, physician notification, and implementation of interventions to address skin breakdown and prevent worsening. On admission, the resident’s record showed a moderate risk for pressure ulcers, but the initial skin documentation did not include wound measurements, detailed descriptions, or physician notification for the skin concerns that were later identified. A skin condition report documented an open pressure ulcer to the right rear thigh, redness to both buttocks, and a recurring rash to the right front thigh, but no measurements or detailed wound descriptions were recorded. The resident’s family member stated staff did not notify them about the pressure ulcers until a couple of days after admission and that turning and repositioning were not done well. The DON confirmed treatment was not in place until the wound nurse assessed the resident, and the Medical Director stated he was not notified of the wounds on admission.
