Summary

The facility failed to maintain complete and accurate medical records for one resident reviewed for admission and medical records. The resident was admitted from a hospital after treatment for cellulitis and abscess of the right leg following a fall at home. The hospital discharge paperwork documented transfer to the facility for PT and OT, but the facility census record, MDS, and other documentation contained conflicting admission dates, and the resident’s admission was not accurately reflected in the record. The record review and interviews showed the resident’s MDS was completed with incorrect information, including an inaccurate admission date and an incorrect statement that the resident had no falls prior to admission, despite hospital documentation showing the resident had fallen at home before hospitalization. The resident’s BIMS was also documented inconsistently, with one assessment showing a score of 15/15 and another psychotropic evaluation documenting 11/15. Staff confirmed these entries were made in error, and the Social Service Director stated the resident was alert and oriented and the lower score was inaccurate. The record also lacked documentation that hospital paperwork was received on admission, that a physician was notified when admission orders were not available, that the hospital discharge medications and treatments were entered into the electronic record and sent to the pharmacy, or that an admission assessment was completed on the actual admission date. The Health Status Note documented the resident as admitted the next day and stated medications were delayed due to pharmacy miscommunication, while the DON confirmed the hospital paperwork was not obtained until the following day and that the admission assessment had not been completed on the actual admission date. The facility policy required accurate, timely, and factual documentation in the medical record.
