The resident was seated in a shower chair when staff turned away to provide privacy or attend to another stall, and the resident attempted to move to a wheelchair that was not locked. The wheelchair rolled, the resident slipped while wet, fell backward, and struck his head on the shower wall. The resident was sent to the hospital and was diagnosed with a subarachnoid hemorrhage. Interviews with the resident and staff confirmed that staff were present but not continuously close enough to prevent the fall.
