Summary

Food was not stored, prepared, labeled, or served in a safe and sanitary manner, and kitchen equipment was not maintained in a clean condition. During a kitchen observation on 05/04/2026, surveyors found several food items with no label or date of opening, including two bags of lettuce, two cartons of strawberries, three pitchers of orange juice, and seven pitchers of apple juice. One bag of lettuce was discolored, and one strawberry carton contained a gray fluffy substance on the strawberry inside the carton. The observation also identified a popcorn machine used for resident activities that was unclean and had kernel residue at the base inside the machine. The same observation found the ice machine used by kitchen staff for resident and staff consumption had a black substance inside it, and when wiped for a clean test, it left a reddish/pink-tinged residue on the paper towel. After the surveyor observation, the ice machine was taken out of service. An Assistant Dietary Manager confirmed the expired, unlabeled, and unclean items, and the Dietary Manager confirmed that food items with no date, no opened-on date, spoiled food, and unclean equipment were unacceptable. The Facility Administrator and DON also confirmed the observations and stated it was their expectation that staff follow facility policies and procedures as written.
