Failure to Maintain Resident Dignity During Care and Communication
Summary
The facility failed to treat residents in a dignified manner by not honoring a cognitively intact resident’s request to use a shower bed and by allowing staff to make non-dignified comments during the shower process. Resident #1 was admitted to the facility, later moved to a room on the Flamingo unit, and had a BIMS score of 15, indicating cognitive intactness. The resident had bilateral upper extremity contractures, impairment on both sides of the body, and was totally dependent on staff for ADLs, including total dependence on one staff for bathing/showering and two staff for transfers. During the interview and observation, the resident stated he wanted a shower bed and said staff were not nice and did not speak nicely or in English. Staff A initially brought a shower chair, told the resident he had a choice of the shower chair or no shower, and said the shower bed was too big to fit, despite the resident’s repeated request for the shower bed and the room having space to maneuver the bed. The Corporate Nurse later agreed to use the shower bed, and the resident was then showered with that equipment. The facility also failed to speak to and treat another resident in a dignified manner when the resident requested assistance with basic care and food. Resident #3, who had a BIMS score of 13, stated staff attitudes were horrible and described being told, in response to asking for help washing her feet, that a staff member had never washed anyone’s feet and was not going to start now. The resident also reported that when she asked for help holding a small plastic cup because it was crumpling in her hand, a staff member responded with profanity. She further stated that when she called the kitchen in the evening for a turkey sandwich, staff replied, “This isn’t Burger King . you can’t get it your way.” A third resident reported that staff spoke Creole in front of English-speaking residents while providing care and were not speaking kindly about residents. Resident #4, who spoke both English and Creole and had a BIMS score of 11, stated staff had a bad attitude and that she witnessed a CNA refuse to give another resident a washcloth to wash her own feet. The resident also reported hearing staff discuss other residents’ bowel movements and transfer needs in Creole, causing concern about what was being said about residents in front of them.
