Failure to Implement QAPI for Infection Prevention and Control

Summary

The facility failed to implement a QAPI plan related to management of the infection prevention and control program. Review of facility policy and documentation, along with interview with the Administrator, showed the QAA committee met monthly and the facility had 12 current QAPI plans in the QAPI binder, but those plans did not include additional QAPI plans that had been initiated before the survey in response to infection prevention concerns identified by surveyors during staff interviews. The Administrator stated she was unaware of a QAPI related to the facility outbreak that an RN had said was implemented in December 2025, and she was not specifically aware of concerns related to the infection prevention and control program. Facility documentation received on 2/4/26 included a PIP worksheet dated 12/29/25 identifying the problem as rapid transmission of respiratory viruses, including COVID-19 and Influenza A, among residents on 2 units. An inservice titled Outbreak Management and QAPI showed 15 of 16 staff signatures were obtained on 12/29/25 and 1 of 16 on 1/14/26, out of 180 staff members. The facility policy stated that all staff are engaged in the QAPI process, that infection control is included under clinical care, that administration is responsible for the QAPI program, and that PIPs should focus on high-risk, high-problem areas using the PDSA cycle and root cause analysis for adverse events such as facility outbreaks.
